Birth, Marriage & Death Registration — Certificates & Extracts
Submit your NIC applications (new issuance, renewal, or corrections) at our dedicated counter. We will certify and forward the forms to the Department of Registration of Persons. One-day and regular services are supported.
Please note that passport applications are not handled at this office. They must be directed to the Department of Immigration & Emigration. A certified extract of your birth certificate from our DS may be required to support your application.

Proceed to the ADR Division counter at Mulatiyana DS within 3 months of the child's birth.

Submit the BHT 30 card issued by the hospital, or a home birth declaration.

Complete the birth notification form with both parents' details and sign the declaration.

The first certified copy is promptly issued on the same day, free of charge.
- BHT 30 card from hospital (or home birth declaration)
- NICs of both parents (originals & copies)
- Birth certificates of both parents
- Parents' marriage certificate
- Child's horoscope (for home births, if available)
Registration within 3 months is entirely free of charge. The first certified copy is also complementary. Any additional copies cost Rs. 100 each.

Submit a completed Form B6 declaration to the ADR Division at Mulatiyana DS.

3 months to 1 year: The ADR can review, approve, and register directly at this office.

Over 1 year: The application is forwarded to the Registrar General's Department for central approval.

Upon approval, the birth is updated in the e-BMD system and a certificate is produced.
- Grama Niladhari certificate verifying the birth details
- Hospital record or BHT 30 card (if available)
- Child's horoscope
- NICs & birth certificates of both parents
- Parents' marriage certificate
- School records or immunisation card (if applicable)
For applicants over 21 years of age, self-declaration is mandatory. Registrations delayed beyond 12 months are sent to the Registrar General's Department, which may involve longer processing times.

Submit the Notice of Marriage to the Registrar at Mulatiyana DS at least 4 days prior to the desired date.

The notice remains on display for 10 days. If no valid objections are raised, the process advances.

Solemnise the marriage legally in the presence of the Marriage Registrar and two reliable witnesses.

The Marriage Certificate (B121) is issued by the Registrar immediately following solemnisation.
- NIC / Passport of both parties (originals & copies)
- Birth certificates of both parties
- Divorce certificate or late spouse's death certificate (if previously married)
- Muslim marriages: Wali declaration (Form E2/3)
- Both parties must be legally over 18 years of age
Kandyan marriages follow a specific statutory procedure under the Kandyan Marriage and Divorce Act. Kindly enquire at our front counter for dedicated guidance.

Obtain the Medical Certificate of Cause of Death (BDR 26) from the attending physician or the hospital ward.

For sudden or unnatural deaths, ensure you acquire the post-mortem report and police summary first.

Present BDR 26 and all supporting documents to the ADR Division counter at Mulatiyana DS.

Upon registration, a death certificate alongside the burial permit will be issued to the informant.
- BDR 26 — Valid Medical Certificate of Cause of Death
- NIC of the deceased (original & photocopy)
- NIC of the informant (the next of kin officially registering)
- Unnatural deaths: post-mortem report & police report
Deaths must be reported without delay. A formal death certificate is critically required before burial and for all subsequent estate matters. Late registrations obligate a Grama Niladhari declaration alongside formal ADR clearance.

Prepare the required application form — B63 for births, or B121 for marriages.

Indicate the Volume and Folio numbers (found on the top-right corner of older certificates) for a rapid search.

Settle the Rs. 100 per copy processing fee at the DS cashier and retain the receipt.

Lodge your request at the ADR counter. Digital e-BMD records are fulfilled same day; archived records may require 1–3 working days.

- Payment receipt — Rs. 100 per certified extract
Please note: The original certificate issued at initial registration is generally not accepted for Ministry of Foreign Affairs attestation. You will need a freshly issued certified extract from our office or the Registrar General's Department for visa processing.

Submit your specific declaration form to the ADR Division at any DS office within the district.

Under 21: Declarations made by a parent or legal guardian — utilize Form B7 (adding name) or B9 (altering name).

21 or above: The individual must declare for themselves — utilize Form B8 (adding name) or B10 (altering name).

The ADR reviews the evidence and authorizes the change. The amended entry is legally recorded and a fresh extract is issued.
